BLUE PRINT GUIDELINE

  1. Summary/Overview
    Form: Narrative
    Required Parts:
    • Mission
    • Vision
    • Core Beliefs/Values
    • Slogan (if applicable)
    • Key Practice Focus
    • Summary of Process
    • Alignment to Regional Strategic Plan
  2. Compelling Need
    Form: Narrative
    Required parts:
    • Research Review
    • Summary of Benchmarks Document points that serve as a focus in the Blueprint
    • Summary of Report Card Data/Indicators that serve as a focus in the Blueprint
    • Summary of HSTW Assessment data
    • Summary of most recent TAV report recommendations
    • Summary of Local Site Data (LSD) sources
    • Rationale for data source selection
  3. Status of the Ten Key Practices and Five Key Conditions
    Form: Narrative and/or Chart
    Required parts:
    • All ten Key Practices
    • All five Key Conditions
    • Current status of each key practice/key condition with supporting data indicators
    • What is the status? How do you know?
    • Desired status of each key practice/key condition with supporting data indicators
    • What should it be? How will you know it has been attained?
    • A description of the gap between the two states--if the gap exists
  4. Goals & Measures for Focus Key Practices
    Form: Narrative
    Required parts:
    • Determination of focus Key Practices including rationale for their selection
    • Statement of goals put into place to Bridge the Gap
    • Data Benchmarks put into place to measure the success of efforts
    • Expected increases in Data Benchmarks over the length of the Blueprint--including rationale
  5. Timeline for Completion
    Form: Chart and Narrative (Bulleted)
    Required parts:
    • List of strategies needed to accomplish goals
    • Chart/graph form for visual presentation
    • Bulleted statements under appropriate year of accomplishment
  6. Team Structure
    Form: Narrative and Chart
    Required parts:
    • Listing of teams needed for the initial implementation phase, including rationale
    • Listing of team membership by roles and responsibilities
  7. Issues and Concerns
    Form: Narrative
    Required parts:
    • Listing of issues and concerns for each of the following development phases (including possible resolution strategy):
    • Blueprint Development Phase
    • Implementation Phase
    • Description of planned communication vehicles necessary for successful completion
    • Listing of further research topics that may need to be investigated, including rationale
